This is an online E log platform to discuss our patient's de-identified health data shared after taking his/her/guardian's signed, informed consent. I have been given this case in order to solve in an attempt to understand the topic of patient's clinical data analysis to develop my competency in reading and comprehending clinical data and come up with a diagnosis and treatment plan. CHIEF COMPLAINT: A 65 year old man who is a laborer by occupation came to OPD with a chief complaint of : -Decreased appetite since 4-5 months. -SOB on exertion since 6 days. -Generalized weakness. HOPI: The 65 yr old male patient was apparently alright 5 months back. Then he had complaints of decreased appetite.
